What is the Delaware County Community Collaborative?
A group of families, people with disabilities, advocates, service providers and community partners working together to:
* Ensure that everyone, regardless of ability, is part of a welcoming community
* Ensure that families with a member with a disability are able to thrive, not just survive
* Help families envision a good life for their family member , and receive the support they need to
achieve their dreams

Sponsored by the Delaware County Office of Intellectual and Developmental Disabilities, under the auspices of the Delaware County Department of Human Services, the Delaware County collaborative works with families and others to:
* Envision a good life for their family member with a disability
* Work with the PA Family Network and service providers to promote the use of the LifeCourse Tools to plan a good life and
a bright future
* Provide training to families, people with disabilities, advocates, community and system partners to increase their
knowledge of the LifeCourse
* Increase community inclusion for all people with disabilities
* Work with stakeholders to maintain people in their communities and avoid out-of-home care
* Work with the PA Family Network and service providers to promote the use of the LifeCourse Tools to plan a good life and
a bright future
